Creating A Polymorphed Creature
Polymorphed creature is an acquired template that can be applied to any willing, corporeal creature (hereafter referred to as the base creature). The base creature takes on the shape of a second creature (hereafter referred to as the assumed form). The assumed creature may be of the same type as the base creature or any of the following types: aberration, animal, dragon, fey, giant, humanoid, magical beast, monstrous humanoid, ooze, plant, or vermin.
Size and Type: Size and type change to match the assumed form. If the base creature had the extraplanar or shapechanger subtypes, it retains them. If the base creature's type was outsider or undead, it remains so.
Hit Dice: Hit dice and hit points are unchanged, but the base creature heals 1 hit point per hit die or character level (whichever is higher) when polymorphing into the assumed form.
The assumed form must have the same or fewer hit dice than the base creature, to a maximum of 15 Hit Dice.
Speed: Same as the assumed form. If the assumed form has magical modes of movement, the polymorphed creature does not gain them.
Armor Class: Same as the assumed form.
Base Attack/Grapple: Base attack is the same as the base creature's; recalculate grapple to reflect the polymorphed creature's change (if any) in size and Strength, and any extraordinary special attacks related to grappling.
Attacks: The polymorphed creature gains the natural attacks and extraordinary special attacks of the assumed form, but not any supernatural or spell like abilities.
Damage: Same as the assumed form.
Special Attacks: The polymorphed creature gains the extraordinary special attacks of the assumed form, but not any supernatural or spell like attacks.
Special Qualities: The polymorphed creature does not gain any extraordinary, supernatural, or spell like special qualities of the assumed form.
Saves: Same as base creature, but recalculate Reflex and Fortitude saves if the polymorphed creature's Dexterity and Constitution have changed.
Abilities: Strength, Dexterity, and Constitution as the assumed form; Intelligence, Wisdom, and Charisma as the base creature.
Skills: Same as the base creature, but recalculate skills with changed ability scores (Strength, Dexterity, or Constitution) or affected by size.
Feats: Same as base creature.
Special: If the base creature has the shapechanger subtype, it can revert to its natural form as a standard action.
